2. Sign-in required
If the creator set "Restrict to users in [organization]" or "Limit to 1 response" (which forces sign-in), respondents must authenticate with their Google account. That authentication links the submission to an identity.
3. Pre-filled links
Form creators can generate a pre-filled link that includes query parameters carrying a specific user's name, email, or ID. Anyone who submits via that link is implicitly identified even if the form itself has no visible identity question.
4. Invitation by email
When a form is sent through Google's "Email" option in the Send dialog, Google logs who received the invitation. Completion tracking through that channel can tie a submission back to the invited address.
5. Third-party tracking
A form embedded in a website can sit inside a page that runs Google Analytics, Meta Pixel, or similar scripts. Those scripts may record a browsing session that includes a form submission, linking it to a cookie profile.
When Google Forms is anonymous
If all of the following are true, responses are anonymous to the form creator:
- "Collect email addresses" is off
- "Limit to 1 response" is off (no sign-in required)
- The form is shared via a generic link, not a personalized pre-filled URL
- The form itself asks no personally identifying questions (name, phone, employee ID, etc.)

Does Google Forms track IP addresses?
Google Forms does not show IP addresses to form creators. The Responses panel and the linked Google Sheet expose only the answers and a submission timestamp. There is no column for IP address, device type, or browser.
However, Google as a company logs every request that hits its servers, including form submissions. That server-level log includes the submitter's IP address. The form creator cannot see it, but Google's infrastructure retains it under standard server logging practices and its Privacy Policy. For most respondents this distinction is not meaningful, but in a legal context a court order or government request could compel Google to produce those logs.

How to make a Google Form more anonymous
Step 1: Turn off email collection
Open the form in edit mode. Click the Settings gear. Under "Responses," verify that "Collect email addresses" shows "Do not collect." If it says "Verified" or "Responder input," change it.
Step 2: Disable the response limit
In the same Responses settings panel, make sure "Limit to 1 response" is off. That toggle requires a Google sign-in, which ties the submission to an account.
Step 3: Share as a generic link
Click Send, choose the link icon, and copy the plain URL. Do not use the Email tab or a pre-filled link generator, both of which can add identifying parameters.
Step 4: Remove identifying questions
Review every question in the form. Delete or make optional any field that asks for a name, employee ID, student number, phone number, or any value unique to one person.
Step 5: Advise respondents on their end
Even with all the above in place, a respondent who submits while signed into their Google account will have the activity logged in their own My Activity. Let respondents know they can sign out of Google before submitting if they need a higher level of anonymity.
Can teachers or employers see who filled out a Google Form?
Teachers and employers who create a Google Form can see respondent identities only if they turned on email collection or required sign-in. If neither is true, and if the form link was shared to a whole class or team rather than to individuals, the creator has no way to match a specific submission to a specific person from the form data alone.
Note that if the form was distributed via a school or company Google Workspace account, a domain administrator may have broader access to activity logs across the organization than an individual teacher or manager would.
