Each theme you describe becomes a block of three to five agreement statements on a five-point scale from Strongly disagree to Strongly agree, with one reverse-worded statement per block so careless answers stand out. Can I generate the survey in another language? Yes. Pick the language in the options and every question, option and scale label is written in it. Spanish, Portuguese, Filipino, Indonesian, Vietnamese, Hindi and Arabic questionnaires come out in the same structure. Can I upload my research proposal or lesson notes? Yes.
