# SOP Quiz: Create a Knowledge Check From Any Procedure

In short

An SOP quiz is a short knowledge check that confirms staff understood a standard operating procedure after reading it or being trained on it.

It is written for trainers, supervisors and process owners who need proof that a procedure was understood.

## Why reading an SOP is not enough

A signature on a read-and-acknowledge sheet says only that someone opened the document. Procedures are often long and written for completeness, so the one step that matters can sit in paragraph six. A quiz forces a second look at the parts that carry risk and shows you which steps people misread. That is useful feedback for whoever owns the SOP: if half the team misses the same question, the wording of the SOP may be the problem.

## Steps to build the quiz

1. **Pick one procedure at a time.** A quiz that spans five SOPs shows nothing useful about any of them.
2. **Mark the critical steps.** Look for steps where order matters, where a number or limit applies, where a check must happen, and where an exception changes what to do.
3. **Choose a question for each.** Use the table below to match the step type to a question format.
4. **Add two scenario questions.** Describe something going wrong and ask what the person does. Real incidents or near misses make the best material.
5. **Write the answer key and a reference.** Each answer should point to the section of the SOP, such as "Section 3, step 5."
6. **Generate a draft from the document.** Use the Formswrite steps below, then read every question and remove anything the SOP does not state.
7. **Publish and track.** Add name, role and SOP version as the first fields. Send it to the group and download the responses.

## Matching step type to question format

| What the SOP says | Question format | Example |
| --- | --- | --- |
| A fixed order of steps | Multiple choice, "what comes first" | Which step comes before sealing the container? |
| A limit or number | Multiple choice or short answer | What is the maximum hold time? |
| A required check | True or false | The second person's sign-off is optional. (False) |
| An exception | Scenario question | The label is missing. What do you do? |
| Roles and who to call | Matching | Match each issue to the person responsible |

## Example: a cleaning and changeover SOP

Imagine an SOP for cleaning a machine between product runs.

**Question 1.** What must be done before you start cleaning the machine?

- A. Remove the guard
- B. Isolate the power and apply the lock as the procedure states (correct)
- C. Run the machine empty for ten minutes
- D. Wipe down the outside

**Question 2.** True or false: the changeover checklist is signed by the operator only. False, it also needs the supervisor sign-off in this example.

**Question 3.** During cleaning you find residue from the previous product in a hard-to-reach area. What do you do?

- A. Continue and note it later
- B. Stop, report it and follow the re-clean step before release (correct)
- C. Ask the next shift to handle it
- D. Release the line if the area is not visible

**Question 4 (short answer).** List the three records you must complete before the line is released.

Replace the content with your own steps. These are illustrations of the format, not instructions for any real machine.

## Using results to improve the SOP

After the first group takes the quiz, sort questions by how many people missed them.

- **Over half missed it.** Revisit the SOP. Is the step buried, ambiguous or in the wrong place?
- **A few missed it.** Follow up with those people individually.
- **Everyone got it.** Keep the question if the step is critical, otherwise swap it for a harder scenario.

Update the SOP and the quiz together. Record the SOP version on the form and retire questions for older versions.

## Build it in Formswrite, step by step

This is the path for a process owner or trainer starting from the SOP as a PDF or Word file.

1. Open the [PDF to quiz maker](/tools/pdf-to-quiz) and upload the SOP as a PDF or Word file. It accepts a PDF, a Word file or a Google Doc, or you can type a topic instead.
2. Choose the question types you want, such as multiple choice, true or false and short answer, and generate the quiz. It comes with an answer key.
3. Read every question. Fix wording, delete anything your source does not state and check each marked answer.
4. Edit the answer key until every question has exactly one clearly correct answer.
5. Share the quiz link, or export it to Google Forms or your learning platform. Exports are part of the paid plans, and the free plan lets you create 5 Formswrite Forms a month. A Google Form is the simplest place to host it: it grades itself and can show the right answers after submission. To write the questions by hand first, see [creating a quiz in Google Docs](/blog/how-to-create-a-quiz-in-google-docs). Regulated industries should follow their own documentation rules for training records.

## FAQ

How often should staff retake an SOP quiz?

Whenever the SOP changes in a way that affects the work, and on whatever refresh schedule your policy sets. Use a fresh mix of questions each time.

Can AI write an SOP quiz?

It can draft one from the SOP text. A person who knows the process must check every question and answer, since a wrong answer key teaches the wrong thing.

What is the difference between an SOP quiz and a knowledge check?

They overlap. A knowledge check is any short test of understanding, and an SOP quiz is one built from a standard operating procedure.

Where should I store the results?

Keep them with the training record for that SOP version, in line with your own documentation rules.
