# Math Quiz Template for 4th Grade With Sample Questions

In short

A 4th grade math quiz template should cover four strands in one short sitting: multiplication and division, place value, fractions, and geometry or measurement. This page gives you a 15-question template with answers, a table showing points and time, and the steps to turn it into a self-grading quiz for a Google Form or a live class game.

## The template

Quiz length: 15 questions. Time: 20 minutes. Total: 20 points (questions 1 to 10 are one point each, questions 11 to 15 are two points each). Calculator: not allowed.

**Part A: Number and operations (questions 1 to 6)**

1. 6 x 8 = ? A) 42 B) 48 C) 54 D) 56. **Answer: B**

3. 3/4 + 2/4 = ? A) 5/8 B) 5/4 C) 6/4 D) 1/4. **Answer: B** (accept 1 1/4)

**Part C: Geometry (question 10)**

1. An angle that measures exactly 90 degrees is called a: A) acute angle B) obtuse angle C) right angle D) straight angle. **Answer: C**

**Part D: Multi-step and word problems (questions 11 to 15, two points each)**

1. A rectangle is 9 units long and 4 units wide. What is its perimeter? **Answer: 26 units** (2 x 9 + 2 x 4)
2. A rectangle is 6 units long and 5 units wide. What is its area? **Answer: 30 square units**
3. A box holds 24 pencils. How many pencils are in 7 boxes? **Answer: 168** (24 x 7)

5. Three friends share 36 stickers equally. How many does each friend get? **Answer: 12**

These match the common 4th grade standards for multiplication, place value, fraction equivalence and area, but check them against your own state standards and pacing guide before you use the quiz for grading.

## Structure table

| Part | Questions | Points each | Skill | Time |
| --- | --- | --- | --- | --- |
| A | 6 | 1 | Operations and place value | 7 min |
| B | 3 | 1 | Fractions | 3 min |
| C | 1 | 1 | Geometry | 1 min |
| D | 5 | 2 | Word problems and measurement | 9 min |
| Total | 15 | 20 | | 20 min |

## Writing good wrong answers in math

Wrong options should be answers students get from a specific mistake. In question 3, 513 comes from forgetting to carry, and 633 from carrying twice. In question 4, 543 comes from a borrowing error. When you can name the mistake behind each distractor, a wrong answer tells you what to reteach.

## Turn the template into a self-grading quiz

1. Type the questions and mark the answers in a Google Doc. Put symbols in plain text (x for multiplication, / for fractions) so they carry over.
2. Open the [math quiz generator](/tools/quiz-generator/math) and paste or upload the file, or describe the topic and grade and have it draft questions.
3. Check the number answers in the preview. Fix any fraction formatting and confirm that each marked answer is the correct one.
4. Export. The free plan includes 5 Formswrite Forms per month. A Google Forms export is paid, and so are the LMS, Kahoot, Quizizz and Blooket formats.
5. For a live review, run it as a timed game with Formswrite Play.

## FAQ

How many questions should a 4th grade math quiz have?

Ten to fifteen is enough for a single class period. Fewer than ten does not cover several skills, and more than twenty tires students who work slowly. Cover one or two strands if the quiz is a short midweek check.

Should a math quiz use multiple choice or written answers?

Use multiple choice for quick fact and concept checks and short numeric answers for computation. Word problems are better as written numeric answers, because a multiple choice list lets students work backward from the options.

Can students type fractions in an online quiz?

Yes, as plain text like 3/4, but accept more than one form of the answer if you grade by text match. For fractions, multiple choice avoids the problem.

Is this template aligned to Common Core?

The topics are typical for grade 4 and many state standards, but alignment differs by state and by district pacing. Treat it as a starting point and compare it to your own standards list.
