# Fillable PDF to Google Form: Convert an Application Form

In short

A fillable PDF is a form people download, fill in, save and email back. A Google Form replaces that loop: people open a link, answer, and every response lands in one sheet. To convert one, upload the PDF to a converter that reads its form fields, check how each field was mapped, and export.

## Fillable PDF to Google Form: Turn a PDF Application into a Form

## Why switch from a fillable PDF

- Responses arrive in one place instead of a pile of email attachments.
- You can mark questions as required, so nothing comes back half empty.
- Nobody needs a PDF reader that supports form fields. A phone browser is enough.
- You can see summary charts of the answers without retyping them.

## How the fields are mapped

A fillable PDF stores its fields as real objects with names and types, which gives a converter much more to work with than a flat page. The [PDF to Google Form converter](/convert/pdf-to-google-form) reads the text, layout and form fields and maps them to matching field types:

| In the PDF | In the Google Form |
| --- | --- |
| Text field | Short answer |
| Multi-line text field | Paragraph |
| Radio buttons | Multiple choice |
| Check boxes | Checkboxes |
| Drop-down list | Dropdown |
| Required field flag | Required question |

Field labels are kept, and required flags are transferred.

## What has no Google Forms equivalent

Some parts of a paper-style form do not translate, so decide what to do before you launch:

- **Signature boxes.** Google Forms has no signature field. Use a short answer asking for a typed full name with a statement of agreement, or collect a signed file with a file upload question.
- **Date pickers and formatted numbers.** Use a date question for dates. For phone numbers or IDs, use a short answer with response validation.
- **Complex tables and matrices.** They may map to a grid question and deserve a check.
- **Attachments the form asks for.** Use a file upload question, which requires respondents to sign in to a Google account.

## Steps

1. Open the PDF in a reader and note any field you rely on, such as a conditional section.
2. Upload it to the converter.
3. Compare the preview with the PDF, field by field. Fix labels and option lists.
4. Mark which questions are required.
5. Export to Google Forms, or keep the Formswrite form if you do not need Google's response sheet.
6. Add a confirmation message, and test by submitting a sample response.

If the PDF is scanned paper rather than a fillable file, OCR reads the printed text and rebuilds the questions, so a clear, high-resolution scan gives the best result. Our guides on [converting a PDF form to a Google Form](/blog/convert-pdf-form-to-google-form) and [converting a PDF to a fillable form in Google Docs](/blog/how-do-i-convert-a-pdf-to-a-fillable-form-in-google-docs) cover the older manual routes, and [can you convert a PDF to a Google Form](/blog/can-you-convert-pdf-to-google-form) answers the general question.

## Example: a club registration form

Say your PDF has a name field, an email field, a drop-down for age group, two check boxes for the activities a member wants to join, and a signature line. After conversion you would expect one short answer for the name, one for the email, a dropdown for the age group, one checkbox question listing both activities, and nothing for the signature. You then add the typed-name agreement yourself and set the name and email questions as required. Total time is usually a few minutes, mostly spent checking the labels.

## Privacy and responses

Forms that collect personal data, such as IDs, medical details or children's names, deserve extra care. Ask only for what you need, say why you need it on the first section, and restrict who can see the response sheet. Check your organisation's rules before moving a sensitive form out of a controlled system.

## Free or paid

Converting and previewing is free, within 5 Formswrite Forms per month, and the Formswrite form is the only free export. Exporting to Google Forms needs a paid plan.

## FAQ

Can I convert a fillable PDF to a Google Form?

Yes. Upload the PDF to a converter that reads form fields. Text fields, radio buttons, check boxes and drop-downs map to matching Google Forms question types, and required flags carry over.

What about the signature on my PDF form?

Google Forms has no signature field. Ask for a typed name plus a statement of agreement, or use a file upload question to collect a signed copy.

Will the Google Form look like my PDF?

No. A form is a list of questions in sections, not a page layout. Logos and page design are not reproduced, but you can add a header image in Google Forms.

Can I convert a scanned paper form instead?

Yes. OCR reads the printed text from a scan and rebuilds the questions. Use a clear scan, and proofread the result.

Do respondents need a Google account?

Not to fill in a normal form. They do need to sign in if you include a file upload question.
